Once the prescription has been sent to your nominated pharmacy by your GP, our staff can then follow up, keeping your medication up to date and thereby safeguarding your health. Gracious Care staff would then transport the prescription immediately from the pharmacy, following whatever system is in place at the pharmacy. Where necessary, they would produce proof of identity and their ID badges are usually suitable. The medication is then immediately taken to the home of the customer and shown to the customer or a relative to ensure that it is correct. The medication is then placed in a safe place, as agreed with the Customer, relative, trusted friend, or other staff.
Staff from Gracious Care can provide support to enable safe self-administration and/or collection of prescriptions or medication when required. Also, where required, a prompting for medication service might be all that is necessary to keep our customer safe.
